Hi Peter,
It's probably smrsh. Sendmail won't allow itself to run an app unless you
add a symlink in the smrsh directory (typically /etc/smrsh on Red Hat). Try:
ln -s /usr/local/bin/lsv_amin /etc/srmsh/lsv_admin
