pppstats -w 1 | awk '{print $1" "$10"\n"}'
Probably won't work coz I can never remember awk syntax. Play with the
"cut" command too.

Configure your modem to return that info in the "CONNECT" string and
look at /var/log/messages if you have debugging on..

I'm not at a machine with a modem right now so take the above as simply
a starting point.
