It looks like at least one of your hard disk partitions are badly damaged.
Have you been shutting down your machine incorrectly (as in, not typing "init
0" or "shutdown") ? It could also be that the hard disk partitions weren't
setup properly & have overlapped.
Either way, it looks like the best thing to do is reinstall - you don't know
what files have been damaged etc.
