What exactly do you want to do ? Do you want a Perl script which writes
a dynamic page i.e. one which changes depending on the value of certain
inputs to the Perl script ? Or do you want the Perl script to write
a web page with a form with some variables whose initial values are
set by the script ?

I'm sorry to not be more helpful, but your question was a bit of an
"I'm sick Doctor" kind of thing - no real clue as to what's required.

If you can't explain it better, perhaps post some example input and desired
output.
