it's a long shot but are you using the apache to do proxying and/or
squid as a httpd accelerator ??
as squid does definately truncate an entry's display based on whatever
value is in the conf & I believe that apache does something similar ?

also & possibly more probable if you're just going plain through a proxy
& not directly accessing local hosts this can arise dependant on your
proxy config.
