Check the permissions on /var/spool/mail itself:
avalon:~$ ls -ld /var/spool/mail
drwxrwxr-x 2 root mail 1024 Oct 18 21:31 /var/spool/mail
avalon:~$
This is on a RH5.2 box. I remember many times having to fix
the owner and permissions on this directory on older Red Hat
releases.
