It's the supermicro SP301-RA [1] as supplied with their SC-760A case
[2].

I'll have a chance to test with another AGP card, and a PCI card also,
to test my hypothesis - if the system comes up with one of the other
cards, I'll test the GeForce in another motherboard, and see where I
am then. I'm not going to give up my DDR GeForce easily 8)
