When you add a new user just restart Samba and it'll find the new user.
(either through /etc/rc.d/init.d/samba restart, or through some signal
sent to smbd and nmbd)

Yep.. IP masquerading is what it's called. Check www.linux.com for some
info on it.

Too many ways to mention. php, perl, C, etc. They'll talk to Access
through an ODBC driver.
Openlink software do some commercial ODBC stuff but you should find good
free/open source ones too.

Not hard, http://cork.linux.ie/projects/install-sendmail/ will have you
up and running before you know it! (wonder if I can set up a procmail
script to search for the word Sendmail and auto-reply ;)
