When pressing 's', you're prompted for "Save to file:" .. just specify the
path there! There's nothing overly complicated about that.
What you're talking about AFAIK was discussed on the Mutt mailing list and has
been added to the Wishlist. In next versions of mutt, there should be a
dir_save_attach option.
