What does kmail launhc to view the email?  Mozilla, Netscape, Opera,
Galeon, Konqueror?

You probably need to configure it to use the proxy server also.  either tha
or you could configure Kmail to launch URLs in Opera:

opera -newwindow '%s'

I don't use kmail, but I'll bet you can do this.
