Still no errors in the log. Graphics card is a Voodoo Banshee, 16mb.
Monitor is a Philips 105E. I set it up manually - Horizontal Refresh is
35-54 and Vertical Refresh is 50-120 (from the manual).

I've tried it from 640x480 up to 1024x768 (what I usually have it at) and
at ranges from 8bit to 24bit - all give the same result.
