I have seen this before on Eircom and Indigo accounts.
As far as I know it usually something in the login script for pppd, in
/etc/ppp/
Not sure what sort of login they are using, probably chap or mschap, but
is you tail -f /var/log/messages and try to dial up you should see the
attempt and failure to log in.
