Sounds like a margin problem.

We had some problems with double page printing through kprinter. It sounds 
your problem might be related.

On Kprinter the double page option on the first of the properties 
configuration just didn't work but the "multiple pages per sheet filter" 
using psnup was OK. However you had to set the margins back on the 
configuration-general tab or else you got text chopped off the edges.

kghostview should automatically use kprinter, if not you can just send any 
file to kprinter instead of lp and get the kprinter menu.

Are you sure the page size A4?
