I used your configuration on a Redhat + windows 200 install no problems so 
I went back and re read the complete thread for any enlightenment.

You copied the partitions with dd. This would only work if both new disk 
and old disk have the same number of heads and sectors in the bios. This 
may explain why linux booted but windows failed. The number of heads and 
sectors is hard coded into the NTFS Boot sector.

Try to copy the partitions from the old disk to the new disk by using a 
partition copying utility which supports copying of NTFS partitions.

Make sure that the windows partition is the first primary partition and 
marked as active. This is very important as NTFS Boot Record code rereads 
the MBR partition table entries to try and determine the partition size in 
sectors.

your lilo.conf is correct assuming that your systems bios supports lba 
mode. Else make sure that both boot sectors reside under the 1024 cylinder 
limit.
