A simple script should do. The following is untested:
find /large/dir |
while read name; do
new_name=`echo $name | sed 's/$name/$new_name/g'`
[ "$name" != "$new_name" ] && mv $name $new_name
if [ -f "$new_name" ]; then
sed 's/$name/$new_name/g' < $new_name > /tmp/rename.$$
mv -f /tmp/rename.$$ $new_name
fi
done
