Can anyone recommend a script / program / solution for managing employee
time-off / holidays?

Basically looking for something which allows employees to request time off
via some form of web interface. Emails management / HR with request and
emails $employee with reply.. 

More bells and whistles would be nice, but I'd settle for something
functional

Don't care what platform it runs on as long as it works

OSS would be preferable, but I don't mind paying for something that makes my
life easier
