My route would be:
pdf2ps -> cat > big.ps -> vi -> ps2pdf
Within vi you modify the initial
%%Pages: 5
and the individual
%%Page: 1 1
to
%%Page: 2 2
etc. You probably need to take out the individual
%%Pages: 1
directives. Use gv or evince to view the edited ps.
