Permissions? Is the procmail process running under the UID and GID of
$USER, and does that user have permission to create directories in
/var/mail/?

I don't know how procmail works - I use postfix-virtual as my delivery
agent, and a static UID and GID rather than maps for specific users. The
/var/mail directory is writeable by my static UID and GID, and
postfix-virtual then auto-creates the relevant directory structures. When
the permissions on /var/mail are incorrect, the subdirectories are not
created.
