A1: that's "init 0" or "shutdown -h $(sometime)".

Others have already answered pretty thoroughly, but my answer:
A2: Have no users on the system that accept interactive logins during
normal operation, from anywhere. Have no processes that can be tricked
into running arbitrary commands, particularly as UID0.
