My money is on a race condition at boot time and udev not being able to
create those devices in time, have you checked the full kernel output for
bugs?
Also check what is in:
/proc/sys/kernel/hotplug
/sys/kernel/uevent_helper
Next, debug the init script calling this, you should be able to adjust the
sleep/delay time if your box is mentally challenged. Last but not least,
is that sitting on any kind of "exotic" RAID controller?
