I'd say it's almost certainly either a hardware problem or a software
problem. Try getting a reproducible fail, and see if anyone else gets
it. If not, then start disabling drivers/removing pieces of kit until it
doesn't break any more, and either stop using the driver that breaks
stuff, or stop using the peiece of kit, or replace them with stuff that
doesn't break.
