At a guess, I'd say that the modem (or serial port) boot in an odd
state, and that the shutdown script which gets run on disconnect
return it to a normal state.
Therefore the first dialup would fail, would return the modem/port
to a normal state on disconnect, and all future attempts would
work.
So, check out the modem or line commands in the shutdown script
and possibly add them as a precursor to the connect also.
Just a guess though ;-)
